Start reading stuff from afar. Like a few feet ahead, or across the room.
Your eyes will learn to adapt.

In short you need to maxamise your time looking at distant things and reduce(but no eliminate you time looking at thing that are closer).

Increase font size on your pc and start sitting farther away from it.
Reduce mobile phone usage to near zero.
Spend time outside in sunlight.
But this will have to be lifestyle change not something you can fix once and go back to bad habbits later.
